Please make sure that you've read through the Pulsera Sale Guide and spent a significant amount of time thinking about the specifics of your sale before filling this out. Please be as detailed as possible when describing your plan.

 

Remember that it is extremely important to send everything back within a day or two of your pulsera sale ending, otherwise another school may miss out on their sale.  If for any reason you need to change the dates, please let us know ahead of time!
